If you’ve already scheduled your intake meeting, here’s everything you need to prepare and what to expect.
Applied Behavior Analysis (ABA) is a science-backed approach that focuses on meaningful behavior change and skill-building across environments. At Evner, we personalize each treatment plan to meet your child’s specific strengths, challenges, and goals.
Step 1: Download and complete the Intake Form
Step 2: Submit the following documents prior to your intake meeting:
Your child’s Individualized Education Plan (IEP) or previous treatment plans
Any psychological evaluations or testing reports
A copy of your child’s insurance card (front and back)
During the intake meeting, we’ll:
Review your child’s IEP and psychological testing (if provided)
Discuss your family’s priorities and treatment goals
Explore therapy recommendations and discuss service models
Review tuition, insurance coverage, scholarship options (including FES-UA)
Schedule an assessment if services are a good fit
