New Parents

We are thrilled to be considered in joining your child’s team! Once you have scheduled an intake meeting, there are necessary items to bring with you to complete the enrollment process. Please use this checklist to ensure a successful meeting!

Enrollment Checklist

Once an intake meeting has been scheduled, the following steps should be taken:

Download and complete the intake form.
Submit the necessary documentation to :
- Individualized Education Plan (IEP) or past treatment plans
- Psychological Testing Report
- Copy of the participants insurance card
At the meeting, previous IEP reports and psychological testing are reviewed to determine the parent goals and potential treatment plans. Tuition and charges will also be discussed.
If parents wish to proceed with enrollment, a student assessment will be scheduled.
A $100 Application Fee after the intake meeting will be charged. Enroll for the Family Empowerment scholarship if applicable.
